No lo sé, jamás la he utilizado.
Sólo supongo que sirve para depurar otras aplicaciones, bajo la experiencia de ver cómo, en ocasiones, el depurador de Delphi interviene cuando alguna aplicación falla. Si puede intervenir cuando falla, ¿por qué no por orden expresa del usuario?
Pero remitiéndonos a la fehaciente ayuda de Delphi:

Choose Run|Attach to Process to debug a process that is currently running. A list of processes running on the local computer is displayed. Select a process from the list and either double-click it or click on Attach to start debugging. The debugger is ‘attached’ to the process.
You are not allowed to attach to a process you are already debugging, nor are you able to attach to the IDE itself.
Remote debugging
On Windows, you can also list processes on a remote machine by entering the remote computer name in the Run|Attach to Process dialog box and clicking Refresh. The remote debug server must be running on the remote computer.
